Step 2:
Attend your appointment
Just arrive at your chosen time with your ID, glasses (if you wear them), and any relevant medical information. Your assessment takes around 15–20 minutes and is carried out by a GMC-registered doctor.
Step 3:
Send your form to DVLA
Once your examination is done, we complete your D4 form on the spot and give you a paper copy to take away. You’ll be ready to submit it to the DVLA immediately — no delays, no extra steps.
Whats Included?
Full Vision Assesment
We carry out a DVLA-compliant eyesight check, including distance and visual acuity testing. If you wear glasses or contact lenses, we test you both with and without them to ensure you meet DVLA standards.
Blood Pressure & Health Screening
Your blood pressure is measured accurately using medical-grade equipment. We also screen for any underlying health conditions that could affect your fitness to drive safely.
Medical History Review
We go through your past medical issues, medications, surgeries and any conditions relevant to driving. This helps ensure your D4 form is completed accurately and meets DVLA requirements.
D4 Form Completion & Certification
Your DVLA D4 medical form is fully completed by a GMC-registered doctor during the appointment. You’ll receive a paper copy before you leave, ready for submission to the DVLA.
